Services may include medical evaluations, follow-up visits, chronic condition management, prescription oversight, and provider-ordered diagnostic testing when clinically appropriate.

This plan is designed to deliver medical services that may qualify as medical care under Internal Revenue Code Section 213(d). Depending on your employer benefits program, certain services may be eligible for reimbursement through Health Reimbursement Arrangements (HRA), Health Savings Accounts (HSA), or Flexible Spending Accounts (FSA).

Itemized medical service invoices may be available upon request to assist with reimbursement submissions. Reimbursement availability is determined by your individual benefits plan administrator.

Designed to support employer-sponsored HRA reimbursement programs.

The Mobile Care Health Telehealth Medical Care Plan provides access to licensed medical providers through a secure telehealth platform designed to support preventive care, diagnosis, treatment, and ongoing health management.

This plan is structured to provide telehealth medical services that may qualify as medical care under Internal Revenue Code Section 213(d) when delivered by licensed healthcare professionals.

Depending on an individual’s employer benefit design, certain services may be eligible for reimbursement through Health Reimbursement Arrangements (HRAs), Health Savings Accounts (HSAs), or Flexible Spending Accounts (FSAs).

Mobile Care Health delivers convenient, provider-led medical care without the delays often associated with traditional healthcare systems.

Itemized invoices for medical services may be provided to support reimbursement submissions to benefit plan administrators.

Medical Services Included

The Telehealth Medical Care Plan provides access to licensed medical providers who deliver virtual medical services, including:

Telehealth Medical Consultations

  • Initial medical evaluation and treatment planning
  • Diagnosis and treatment of acute medical conditions
  • Chronic disease management consultations
  • Preventive health evaluations 

Telehealth visits are documented in the patient’s medical record and delivered by licensed healthcare professionals practicing within their scope of licensure in states where telehealth services are permitted.

Ongoing Clinical Care

Patients may receive ongoing clinical care, including:

  • Follow-up telehealth visits related to treatment plans
  • Medication management and prescription review
  • Health monitoring consultations
  • Medical care coordination when appropriate 

These services allow clinicians to provide continuity of care and support long-term health management.

Diagnostic Medical Services

When ordered by a licensed medical provider, diagnostic testing may include:

  • Routine laboratory testing
  • Metabolic and cardiovascular risk assessment testing
  • Hormone or endocrine testing when clinically indicated
  • Other medically necessary diagnostic services 

Diagnostic testing is ordered by clinicians when medically appropriate and billed separately from telehealth medical services. When provided as part of clinical care, diagnostic testing may qualify as medical care under IRC §213(d).

Remote Patient Monitoring (Optional)

Some patients may participate in Remote Patient Monitoring to allow clinicians to review health metrics between telehealth visits.

Monitoring may include metrics such as:

  • Blood pressure
  • Body weight
  • Heart rate
  • Sleep metrics when clinically appropriate 

To ensure secure integration with our electronic medical record system, Remote Patient Monitoring requires the use of approved connected devices compatible with our clinical monitoring platform.

Patients who participate in Remote Patient Monitoring must purchase compatible devices through Mobile Care Health to ensure proper integration with the monitoring system. Devices are billed separately as medical equipment and are not included in the monthly telehealth medical services fee.

What Is Not Included in the Medical Services Fee

To maintain clear separation between medical services and non-medical benefits, certain services are not included in the telehealth medical services portion of the plan and may be offered separately when applicable. These services may not qualify as reimbursable medical expenses under IRS guidelines.

  • Consumer health devices
  • Supplements or wellness products
  • Discounts on elective treatments or services
  • Longevity testing or specialty wellness programs

Telehealth Medical Access Plan

$199 per employee per month

  1. Annual virtual exam 
  2. Two routine visits per year
  3. Two telehealth urgent care calls per year.

Telehealth Primary Care Plan

$299 per employee per month

  1. Annual Virtual Exam 
  2. Four Routine Visits per year
  3. Clinician-Directed Lifestyle Counseling – Four 30-minute visits per year
  4. Sleep Evaluation Consultation
  5. Eight Telehealth Urgent Care Calls per year
  6. Bi-monthly Follow-Up Visits

Remote patient monitoring, diagnostic testing, and medical equipment may be recommended by clinicians when medically appropriate and billed separately.

HSA / HRA Reimbursement Notice

Certain services provided through our telehealth plans may qualify as eligible medical expenses under IRS Section 213(d).

Patients may be able to submit itemized invoices for reimbursement through:

  • Health Reimbursement Arrangements (HRA)
  • Flexible Spending Accounts (FSA)

However, eligibility for reimbursement depends on the individual’s employer benefit design and applicable IRS regulations. Mobile Care Health does not guarantee reimbursement.
